The Florida Supreme Court has released a new revision to the Florida Addendum to Income Withholding Order form 12.996(d) that courts use in many family law cases. This is a supplemental form that is used together with the federal Income Withholding for Support form. These two forms are often used to require employers to withhold child support and alimony payments directly from an employee’s paycheck.
The primary change is to the instructions to help people complete the federal form (it can be confusing, especially if you are not a lawyer). There was also a minor typo that was corrected.
What It Means For You
No changes are needed if you already have a signed order. If you are in the process of requesting an Income Withholding Order from a judge, use the revised form. The latest form can be found at the Florida State Courts website.
